Not only do two of the Fifth Doctors companions die, two of them were plotting to kill him for different reasons at the same time, and one of those he himself kills.

I only remember Turlough plotting to kill 5, and only because the Black Guardian promised to return him to his homeworld.
He couldn't save Adric, which is why the boy died.

The Doctor should never travel alone. He either needs someone to show off for, or who will yank him back from doing something stupid (or gleefully join him).
